Software Installation Request Form (SIRF) / Software Continuance Form (SCF)

This form is not to be used for NEIU Faculty / Staff computers. For software install requests for NEIU Faculty / Staff computers, please use the Helpdesk Support Request form.

The Software Installation Request Form (SIRF) PDF is used to request course-related instructional software to be added to computers under the control of Student Computing Services (SCS) in either a Technology Enhanced Classroom (TEC) or an Open Computer Lab. Commercial software requires proper licensing and must be purchased by the university. Free / Open Source software must conform to one of the known Open Source licenses. No software will be installed that violates the software’s License, is End-of-Life / Out-of-Support, is considered unstable (alpha, beta, release candidate), does not have an installer or is considered Server software (FTP, HTTP(S), SQL, etc.). VMs or Windows Subsystem apps will not be permitted.

The Software Continuance Form (SCF) PDF is to be used to request that course-related instructional software be kept in its installed location beyond the time request on the original SIRF or to inform SCS that the software is sill being used. To begin the SIRF process:

1. Download the appropriate PDF on this page. Read all the important information on pages 1, 2 and 4. Note the submission Deadlines, General Information and General Policies. Then, fill out the SIRF where applicable.
2. Save your completed SIRF and upload it using the Request Service button at the right.
3. Print out the completed SIRF and obtain the necessary signatures.
4. Print out the licensing terms for the requested software.
5. Put the requested software on a CD, DVD or USB drive.
6. Per the requirements of the SIRF, submit all printed and signed materials as well as the software to room B-105 to the attention of Student Computing Services.

The SIRF process is considered complete when steps 2 through 6 have been completed.

Deadlines

Spring Semester
Software Installation Request Forms and the Software to be installed for a course in an SCS lab or Technology Enhanced Classroom (TEC) must be turned in no later than the last week of November to be available for the beginning of the Spring Semester. Requests for software installations in open labs and TEC rooms must be submitted in advance of the semester in which you plan to use the software as these installations are performed during semester break maintenance periods in order to avoid lab closures or disruptions during normal scheduled school hours. No Software Installation Request forms or software will be processed once the semester has started.

Summer Semester
Software Installation Request Forms and the Software to be installed for a course in an SCS lab or Technology Enhanced Classroom (TEC) must be turned in no later than the last week of April to be available for the beginning of the Summer Semester. Requests for software installations in open labs and TEC rooms must be submitted in advance of the semester in which you plan to use the software as these installations are performed during semester breaks maintenance periods in order to avoid lab closures or disruptions during normal scheduled school hours. No Software Installation Request forms or software will be processed once the semester has started.

Fall Semester
